Fixed a bug where equipping Erebus' Unwavering Punishment could cause you to take extra damage from monsters

Fixed a bug where offhand items were not displaying in the Unique Codex

Further expanded the accuracy of boss life calculations, ensuring that it's even tracked if you deal 1 damage to a 10e38 life boss (please don't try to win a fight like this!)
